Therapy Guidelines

Check the time in and time out. It Should be not less than 45 minutes and not more than 1 hour.

The date of the visit and the therapist’s signature , they should be the same date.

Check the Vital signs.
For normal VS:
systolic BP - 100-160
diastolic BP - 60-90
Pulse or Heart rate -  60-90 bpm
Respiration - 12-20 resp per min 

Note:
Therapist should check VS of the patient before and after the exercises.

4. Check for the pain level.
For pain level:
1-4 is normal does not need to be reported
5 and above is too high should be reported to  the hha immediately.

5. Read comments/important details in the notes. Report to the HHA any abnormalities in the VS and pain level immediately.

Important:
Every therapy progress note must justify medical necessity. The note must show the patient progression from the treatments received. If the therapists sees that the patient will no longer benefit from further therapy,  the patient should be discharged. Or, if the HHA sees no changes in the patients condition, they will report these to the therapists attention.
